Name and address of insured.

Claims/Loss Experience

Previous record and experience.. Three years experience including all losses, claims paid and outstanding.

Type of operation

What type of operator? Haulier, Freight Forwarder etc
Does the insured own or operate vehicles?
Does the insured subcontract work to others?
Does the insured act as an agent issuing shipping and or transport documentation?
What type of operation,
What type of goods carried? General merchandise or specialist,
High or low value, abnormal loads and heavy lifts, etc.

Storage

Is there any storage involved?
If yes what conditions of storage are used
what are the physical attributes and security arrangements at the storage site.

Conditions of Carriage

What conditions of carriage are used? RHA, CMR, FTA etc.
What year of the above standard conditions are used.
Any carryings under All Risks contracts?
Are the limits of liability different to the standard conditions?

Sub Contracting

Does the proposer sub-contract for or to others, if so under what
Conditions is this done?.

Area of Operation

Does the insured operate purely in the UK?
Type of goods carried
Does the insured carry goods to the continent or Ireland?

Method of Transport

Does the proposer have any vehicles?
How many vehicles?
What is the carrying capacity of the vehicles?
What type of vehicles does the proposer operate?
Are they fit for the purpose?
Does the proposer use sub-contractors?

Employees

Does the insured obtain references directly from at least two former employers
Does the insured obtain and keep on file passport type photographs of drivers and other employees responsible for cargoes?
Does the insured employ agency drivers?
Does the insured obtain (Passport size) photographs of drivers/employees?
